We cannot respond to your request or provide you with personal information if we cannot verify your identity or authority to make the request and confirm that the personal information relates to you. The information we request below, and any additional information we may request when reviewing your submission, is to assist us in verifying your identity, fulfilling your request, and responding to your request.

(i) that the information I have provided is true and correct;
(ii) I understand that the information will be processed by the Everise group of companies (collectively, “Everise”) in accordance with its Privacy Policy; and
(iii) I understand that Everise may contact me to confirm the request and/or obtain additional information required to verify the request.
Restricted Information
Please note that we will not be able to comply with your request in certain circumstances:
(i) if we are provided with insufficient information to locate the personal information requested;
(ii) if the request relates to personal data which is commercially confidential or proprietary to Everise; or
(iii) if we are unable to verify the identity of the requestor subject to notifying you.
